Pre Appointment
Do you have a fever or have you felt hot or feverish recently?
Are you having shortness of breath or other breathing difficulties?
Do you have a continuous new cough?
Any other flu like symptoms such as gastrointestinal upset, headache or fatigue?
Have you experienced recent loss of taste or smell?
Are you or have you been in contact with any confirmed cases of Covid-19?
Is your age over 70?
Do you have heart disease, lung disease, kidney disease, diabetes or any auto-immune disorders?

Positive responses to any of these questions would likely indicate further discussion with a clinician before proceeding with dental treatment.
